Carroll Lutheran Village as part of its services to its residents has a medical component. To that end, Carroll Lutheran Village has a HIPAA Compliance Officer who oversees our policies and procedures regarding your Protected Health Information. Prior to your admission to Carroll Lutheran Village, you will receive “Notice of Information Practices”, “Consent to the Use and Disclosure of Health Information for Treatment, Payment, or Healthcare Operations” and “Authorization To Use and Disclose Protected Health Information” forms for your review and signature.
